School Supply List
Required Forms
Please complete medical exams as necessary and submit all required medical forms before the first day of school (students cannot attend school until this requirement is met). All students need the liability form. New students need the Ethnicity form, Physical form, and a copy of their child's birth certificate. Please turn all of these forms in to Mrs. Tres or your classroom teacher.
Liability & Release Form: ALL students (new and returning)
Physical Exam/Immunization record: All students new to ILS (with Tdap booster and MCV vaccines)
Race and Ethnicity Data: All students new to ILS
Parents or legal guardians who are requesting a religious exemption to immunizations or examinations should fill out the Religious Exemption Form.
If your child has an EpiPen or for medication administered during school or during before/after school hours, including but not limited to analgesics and inhalers, ibuprofen, antacid tablets, etc.; please give the completed Medication Administration Form to the school office when dropping off clearly marked medication, with your child's name and instructions, in an appropriate pharmacy or over-the-counter container.
Extended School Supervision (ESS) Registration
If you plan to have any of your students attend ESS this school year, please fill out the registration form. The $50 registration fee will be billed through FACTS as an incidental fee once you complete and submit the ESS registration form. Please complete the form and return it to Mrs. Tres or your child’s teacher.

August 25 - Full Day Preschool and Young 5’s Full Day Prekindergarten Open House & Final Registration 4:00-7:00pm
August 27 - Full Day Preschool and Young 5’s Full Day Prekindergarten Begin
August 27 - Open House & Final Reg. 10:00-11:00am (Woell Prekindergarten) or 6:00-7:00pm (Woell Prekindergarten and Campbell Young 5’s Half-Day)
August 28 - Preschool Open House & Final Registration 10:00-11:00am or 6:00-7:00pm
September 2 - Young 5's Half-Day begins (full session)
September 3 - M/W/F Preschool begins (1 hour session)
September 3 - Prekindergarten begins (full session)
September 4 - T/Th Preschool begins (1 hour session)
September 4 - Young 3’s Open House & Final Registration 4:00-6:00pm
September 9 - Young 3's begins (1 hour session)

You will receive a letter in the mail with your teacher's contact information. She will be reaching out to you to set up a Home Visit before school starts. These will take place after August 15. Please be patient, as she has many children in the class.
