Required Forms

Please complete medical exams as necessary and submit all required medical forms before the first day of school (students cannot attend school until this requirement is met). All students need the liability form. New students must submit the Ethnicity form, Illinois State Physical form, and a copy of their child's birth certificate. Please turn all of these forms in to Mrs. Tres or your classroom teacher.

Illinois State Physical Form/Immunization record: All students new to ILS (with Tdap booster and MCV vaccines) *Physical form must be turned into the office before the first day of school, or your child may not attend. Contact Mrs. Tres with exceptions.

Race and Ethnicity Data: Required for all students new to ILS

Religious Exemption Form: Only parents or legal guardians who are requesting a religious exemption to immunizations or examinations should fill out the Religious Exemption Form.

Medication Administration Form: Only if your child has an EpiPen or for medication administered during school or during before/after school hours, including but not limited to analgesics and inhalers, ibuprofen, antacid tablets, etc.; please give the completed Medication Administration Form to the school office when dropping off clearly marked medication, with your child's name and instructions, in an appropriate pharmacy or over-the-counter container.

Extended School Supervision (ESS) Registration

If you plan to have any of your students attend ESS this school year, please fill out the registration form. The $50 registration fee will be billed through FACTS as an incidental fee once you complete and submit the ESS registration form. Please complete the form and return it to Mrs. Tres or your child’s teacher.

You will receive a letter in the mail with your teacher's contact information. She will be reaching out to you to set up a Home Visit before school starts. These will take place after August 16. Please be patient, as she has many children in the class.
