K-8 Required Forms

Please complete medical exams as necessary and submit all required medical forms before the first day of school (students cannot attend school until this requirement is met). Please complete them and submit them to the school office via hard copy or email to hdroz@ilcp.org.

Eye Exam: Kindergarten, students new to ILS

Dental Exam: Kindergarten, Grade 2, Grade 6, students new to ILS

Health Exam/Immunization record: Kindergarten & Grade 6, students new to ILS (with Tdap booster and MCV vaccines)

Race and Ethnicity Data: All students new to ILS

Parents or legal guardians who are requesting a religious exemption to immunizations or examinations should fill out the Religious Exemption Form.

ALL STUDENT ATHLETES require an IHSA sports physical examination form. This form differs from the general physical examination form.

If your child has an EpiPen or for medication administered during school or during before/after school hours, including but not limited to analgesics and inhalers, ibuprofen, antacid tablets, etc.; please give the completed Medication Administration Form to the school office when dropping off clearly marked medication, with your child's name and instructions, in an appropriate pharmacy or over-the-counter container.

K-8 Extended School Supervision (ESS) Registration

If you plan to have any of your students attend ESS this school year, please fill out the registration form. The $50 registration fee will be billed through FACTS as an incidental fee once you complete and submit the ESS registration form. 

  1. Download the attached form. 

  2. Complete.  Sign and date the form.

  3. Save the completed form.

  4. Email the completed form to hdroz@ilcp.org

The class with the highest percentage of students in attendance will win special back-to-school treats in the first week of school! Be sure to check in with your teacher!

Thursday, August 14

  • K-5 Meet the Teacher (signup will be available on August 1!)

  • Red, White, and Scoops Ice Cream fundraiser for D.C. Trip from 10:00am—4:00pm

  • Adopt a book for the library in celebration of your student’s birthday from 10:00am-4:00pm

  • Middle School Locker Setup from 5:00-6:00pm

  • Mandatory parent/student meeting for students interested in ANY athletics programs at 6:00pm (Zoom option available)

  • Fall Athletics Meetings (Volleyball, Cross Country) immediately following general athletics meeting (Zoom option available)

Sunday, August 17

  • Backpacks, Blessings, and BBQ at the Bandshell @ 10:30am

Tuesday, August 19

  • Grades K-8 First Day of School (dismissal at 11:00am, NO ESS)

Wednesday, August 20

  • First chapel of the year at 1:30pm
