K-8 Required Forms
Please complete medical exams as necessary and submit all required medical forms before the first day of school (students cannot attend school until this requirement is met). Please complete them and submit them to the school office via hard copy or email to hdroz@ilcp.org. If forms are not submitted by August 17, Dr. Calendo will contact you to confirm that your student will not be eligible to start school on August 19.
IMPORTANT:
In order for forms and exams to be valid for 2026-2027, appointments must occur AFTER the following dates:
Eye Exam - December 31, 2025
Dental Exam - October 31, 2025
Health Exam - August 18, 2025
Eye Exam: Kindergarten, students new to ILS
Dental Exam: Kindergarten, Grade 2, Grade 6, students new to ILS
Health Exam/Immunization record: Kindergarten & Grade 6, students new to ILS (with Tdap booster and MCV vaccines)
Religious Exemption Form: Parents or legal guardians who are requesting a religious exemption to immunizations or examinations should complete the Religious Exemption Form and have it signed by a health care provider.
NEW: ALL STUDENT ATHLETES may submit a general health examination form in place of the previously required IHSA sports physical examination form.
Race and Ethnicity Data: All students new to ILS
Medication Administration Form: If your child has an EpiPen or for medication administered during school or during before/after school hours, including but not limited to analgesics and inhalers, ibuprofen, antacid tablets, etc.; please give the completed Medication Administration Form to the school office when dropping off clearly marked medication, with your child's name and instructions, in an appropriate pharmacy or over-the-counter container.
K-8 Extended School Supervision (ESS) Registration
If you plan to have any of your students attend ESS this school year, please fill out the registration form. The $50 registration fee will be billed through FACTS as an incidental fee once you complete and submit the ESS registration form.

Meet our new staff!

I am excited to join the Immanuel Lutheran community. I believe deeply in faith and family. I grew up in a loving Christian home with seven siblings. My husband, Tom, and I are blessed with four children and two grandchildren. We enjoy spending time together—hiking, biking, skiing, and traveling.
I graduated from Northern Illinois University with a degree in accountancy and began my career as a CPA. I was blessed with the gift of staying home to raise our children. When our youngest two children attended St. Paul Lutheran School in Mount Prospect, I accepted an aide position and discovered my true calling in the classroom.
I served for thirteen years at St. Paul Lutheran School in roles including classroom aide, middle-school resource math teacher, and resource teacher. In the 2025–2026 school year, I supported 45 students. The greatest reward for me is watching a student overcome a challenge and seeing the pride that shines from their success. It is a priceless opportunity to work with students and the fun and joy they bring to every moment. I look forward to serving the Immanuel Lutheran family with care, faith, and a heart for each child.

Barb Olsson is a follower of Jesus Christ, married to her awesome husband Dave for 36 years, mom to four grown kids, and Grammy to two sweeties! She has been an educator her whole life— teaching high school in New York and junior high in Illinois, and most recently substitute teaching in Crystal Lake.
When she is not visiting her kids, you can find her gardening, tending her chickens, or walking her dog Cooper.
She will be at the Locker Launch to meet MS students and families.
